Sterile Reconstitution Checklist

Always follow strict sterile precautions to prevent biological contamination. Compounding peptides outside a clinical lab requires absolute sanitation:

  • 1Sanitize hands completely with antibacterial soap and wear latex or nitrile gloves.
  • 2Wipe the rubber stoppers of both the bacteriostatic water vial and the peptide vial with fresh alcohol prep pads.
  • 3Inject an equal volume of air into the water vial before drawing fluid to maintain matching pressure.
  • 4Slowly drip bacteriostatic water down the inside glass wall of the peptide vial. Avoid shooting water directly into the powder.
  • 5Slowly roll the vial between your palms until dissolved. **Do NOT shake the vial**, as shaking can denature fragile peptide bonds.
  • 6Store reconstituted Semaglutide in the refrigerator (2°C - 8°C / 36°F - 46°F). Do not freeze.

What is Semaglutide and How Does It Work?

Semaglutide is a revolutionary GLP-1 (Glucagon-Like Peptide-1) receptor agonist. Originally approved for Type 2 Diabetes under the brand name **Ozempic**, its powerful metabolic and appetite-suppressing effects led to FDA approval under the brand name **Wegovy** for chronic weight management.

Semaglutide mimics an intestinal hormone released after eating. It targets receptors in the brain to reduce hunger, prolongs satiety by slowing stomach emptying (gastric delay), and improves insulin sensitivity in response to food intake.

The Official Semaglutide Titration Schedule

To minimize gastrointestinal side effects (such as mild nausea or acid reflux), Semaglutide is strictly administered on a **step-wise, 4-week titration schedule**. Standard medical protocols recommend starting with a low micro-dose and gradually stepping up every 4 weeks until reaching the maintenance dose:

PhaseTimelineWeekly DosePurpose
Step 1Weeks 1 - 40.25 mgBody introduction & adaptation
Step 2Weeks 5 - 80.50 mgMild appetite suppression
Step 3Weeks 9 - 121.00 mgPrimary therapeutic dosage
Step 4Weeks 13 - 161.70 mgHigh-tier metabolic correction
Step 5Month 5+ (Onward)2.40 mgFull clinical weight maintenance

Compounding & Reconstitution Guidelines

Lyophilized peptide vials require adding **Bacteriostatic Water** (sterile water containing 0.9% benzyl alcohol as a preservative) to dissolve the active powder before administration. Since standard compounding options vary (e.g., 2mg or 5mg vials), performing the dilution math is critical to avoid dosage errors.

Reconstitution Calculation Blueprint
Concentration (mg/mL) = Vial Mass (mg) ÷ Water Added (mL)
Syringe Units (U-100) = (Desired Dose (mg) ÷ Concentration (mg/mL)) × 100
